The long term plan for the NHS places emphasis on personalised care and in particular commits to the introduction of social prescribing link workers by 2020/21. On the Fylde Coast we are already working in primary care networks and will be looking to implement more social prescribing over the next year. The question is how? What do the Primary Care Networks need to do and how do the thousands of community organisations on the Fylde Coast fit in?

The Healthier Fylde Coast integrated care partnership is hosting a social prescribing conference on Wednesday 12 June to work through some of those very questions. We will have speakers from NHS England, local experts on patient activation and updates on what is already happening on the Fylde Coast. Then we will be breaking in to neighbourhood groups to talk through how best to move forward on the Fylde Coast.
