What is it all about:

LSCFT were successful in bidding for transformation funding from NHS England to develop new models of integrated community mental health care across Lancashire and South Cumbria.


The Aim:

Together, we would like to develop a new population-based model for mental health care in the community. This new model of care will look to redesign and reorganise the existing community mental health resource to form new community mental health hubs each of which will be aligned to the local GP Primary Care Networks (populations of 30,000- 50,000). The community mental health hubs will have blended teams of health, social care and VCFSE staff; working together, and with local communities, to improve mental and physical health.


What are we looking for:

We would like to hear from VCSFE organisations across Lancashire and South Cumbria about the best ways for us to mobilise this sector funding. Specific areas that we are looking to develop with VCSFE include support for Refugees, Asylum Seekers, Community Connectors, Peer Support, BAME Support Workers and support for people with complex lifestyles.
